All persons affiliated with the University of Michigan Health System are required to wear a current Health System Mcard. Health System Mcards are only issued at the Key & ID Office in the Med Inn Building. If a person requires access to the Health System and Ann Arbor, Dearborn, or Flint campuses, cardholder must obtain Mcard at the Key & ID Office. The Health System Mcard Request and Change form is required for all new and replacement Mcards with the exception of damaged Mcards. The Key & ID Office does not accept card or replacement fee payments. Payments must be made at the Hospital Cashier's Office. The paid receipt is then given to the Key & ID Office.

Students, Faculty, Staff, and U of M Staff Services Temps Please bring government photo ID (i.e., driver's license, state ID, passport) with you. People wearing sunglasses, hats, caps, bandanas, etc. will be required to remove them before their photo is taken. People who have a previous Mcard will be required to turn it in to obtain the new Mcard. Student, faculty, and staff Mcards are valid up to five years, while the UM Staffing Services Temps are valid up to one year. The University department or unit name will print under cardholder's name. Expired Mcards and new Health System Mcard Request and Change form are required to be turned in to the Key & ID Office for new Mcards.
Contractor, Un-Paid Staff, Vendor, Visitor, & Volunteer Mcards These cardholders are required to bring a completed Health System Mcard Request and Change form to Key & ID Office and a paid receipt for $6.00 for an Mcard. Some Health System units sponsor the $6.00 costs with internal billings. Please bring government photo ID (i.e., driver's license, state ID, passport) with you. People wearing sunglasses, hats, caps, bandanas, etc. will be required to remove them before their photo is taken. People who have a previous Mcard will be required to turn it in to obtain the new Mcard. Mcards are valid up to one year. The University department or unit name will print under cardholder's name. It is very important that Mcards be handled with care, similar to the way care is taken with driver's licenses or credit cards. Mcards should last four to five years without failure. It is advised that Mcards not be placed in extreme heat (e.g., left on car dashboard) as the card will warp. Mcards that are turned in to the Key & ID Office because of normal wear and tear are replaced for free. Cardholders could be charged the Mcard replacement fee if it is apparent that the cards were abused from such damage reasons in the red image above and others. Examples of abused Mcards The Key & ID Office provides a cardholder and clip or neck chain for displaying Mcard.
Incorrect or changed names on Mcards require the cardholders to provide proof (driver's license, marriage license, etc.) to an Human Resources for employees, the Registrar's office for students, or the Key & ID Office for all others. Generally after Human Resources or the Registrar's office corrects the record, it takes 20 to 30 minutes to change in the ID Card database. Once the change is updated in the ID Card database, the old Mcard can be turned in to the Key & ID Office for a new Mcard. Cardholders who also become University students have the option to turn in their Mcards to the Key & ID Office and obtain Mcards which have their department listed on the obverse and the word "STUDENT" printed on the reverse above the black magnetic stripe. Cardholders who change departments can bring completed Health System Mcard Request and Change forms and their old Mcards to the Key & ID Office for their new Mcards. Expired Mcards, new completed Health System Mcard Request and Change forms, and paid receipts for non-students/employees are required to be turned in to the Key & ID Office in order to receive new Mcards. Permanently leaving the University requires that the Mcard be turned into the Key & ID Office or to the department/unit supervisor. The supervisor will forward this Mcard to the Key & ID Office. |
